Late one evening, the quiet of my office was suddenly broken. The CEO appeared at my door, looking visibly troubled.

"Come with me, I need you," he said in a tone that left no room for questions.

He led me straight to the production floor, where a tense crowd had gathered around our most complex, high-pressure gas machine. Standing in a tight circle were the Operations Director, the Maintenance Manager, the Department Head, the Shift Supervisor, and the Machine Operator. You could feel the heavy anxiety in the air.

When workers or supervisors propose bypassing a safety safeguard, it is rarely out of negligence or a desire to cause harm. Instead, it is almost always driven by organizational context and misaligned priorities.

When leadership intensely prioritizes deadlines, speed, and production quotas, it creates an environment where workers feel compelled, if not forced,to take shortcuts. If safety is only championed when it is convenient, but discarded the moment a client shipment is delayed, the workforce quickly perceives safety as a mere corporate platitude.

To prevent bypasses, executives must address the upstream pressures and conditions of work rather than just policing frontline behavior. True safety leadership means aligning organizational values so that operational progress never forces workers into making an impossible choice between their livelihoods and their lives.


3. Trust is the Fundamental Currency of Safety

A safety culture cannot function without a deep foundation of mutual trust. If workers fear reprisal or punishment, they will hide mistakes, cover up near-misses, and keep operational bypasses a secret from management. As safety experts frequently warn: we cannot fix a secret.
